Query for creating own email template in Jira service management

Is it possible to create our own email templates with below details in Jira service management, once user fill the details and send the mail based on that it will create a ticket with below details in Jira service management

Based on my experience, you can find what you're trying to achieve using Notification Assistant for Jira Cloud.

It allows to create mail templates to be used in notifications.
