Importing from csv

Hi everybody,
i need to import some activities to Jira from csv file, but i don't kwow how to recreate a structure similar as in pic. I have an epic and many tasks that are dependent on each other and stuck even if the task before is not finished. How can I bring this structure back to the csv file. Thank you
