How to import issue in correct way from Excel/CSV ?

Hi,

I have a excel file (On Attachments). i want to import this excel after i turned into CSV to Jira. But, i have issues when i click on "Next" i only can Mapping 1 table. i assume it represent all these column.

any idea so i can mapping easily ?
