I'm using Confluence Cloud. When I click on Gliffy in the Apps section in the left panel, I see "Gliffy Diagram" in the heading but there are no buttons or menus to create a Gliffy.

Please try to clean you browser cache and try again! If it doesn't work try to contact gliffy team through https://www.perforce.com/support/request-support

You add a Gliffy diagram while editing a Confluence page! The item on the left is more for configuring your app's settings. Here's a link to a full how-to for you
