Needing some guidance.
I have created a custom field, using the Select list (cascading) field type. Whilst in Forms, the custom field isn’t visible. However, its visible in Request Types.
What am I missing?

Based on my understanding, cascade select list data type custom field is currently not supported in the FORM configuration where one can link this type of field to form's field: https://docs.thinktilt.com/proforma/linking-proforma-fields-to-jira-fields
I would recommend that you to contact Atlassian Support (https://support.atlassian.com) to obtain update on when they will fix this limitation.
