How to enable a template

Hi,

I'm trying to setup a new template for a project. I've created a template successfully but when I go to use it I get error saying that I must "Fill in all required fields" I've filled in all the fields with an * and am unsure of how to get passed this step to enable a new template. Is there something I'm missing?

Are you using an app from a vendor? Cause this doesn't look like out of the box jira functionality.
