How to create a request form in Jira cloud?
I need a form requesting a title/description and environment (multiple options) so that after it is submitted a Jira issue is created.
could the "issue collector" simplify this?

Issue collectors may be used for such things you describe. However this collector do not work inside Jira cloud, but via another website. You can learn more about them here https://support.atlassian.com/jira-cloud-administration/docs/use-the-issue-collector/
Another way to raise requests in a form would be in a Work Management Project, where you can use the Form feature. There you create a form and share it with your colleagues. They login into jira and complete the form. Then this form will create issues https://support.atlassian.com/jira-work-management/docs/collect-information-for-your-team-with-a-form/
