Can we add the icon as the option under custom field?

Hi Team,
I need your help on adding the icon as option rather than text under custom field.

✅ Yes
🟥 No
⬛️ N/A

These are my options. I need only the icons as we have it in priority.

I want to add like this. Please help. This will help me in reports for a better view.

You will need to find or write an app that can provide a field that can do this.
