Create JSM request based on a survey/poll

I have a requirement to deploy multi-factor authentication to all remote users for VPN.

A request type and automation rules are configured at the portal.

A large percentage of users are returning to work and I would like to send a poll/survey to verify who would need remote access going forward. 

• Could the questionnaire be sent from JSM?  
• Simple click yes or no collected
• Could a yes click generate a request
