Prepopulate Description for Bug Ticket

I'm pretty new to Jira, and have searched but haven't found specific answers. I want to require the following information in the description of a Bug ticket: 

Problem

User ID

Steps to Recreate or Bird recording link

Expected Result

 

Can I

1. Have those items be in the description for them to fill out? 

2. Create a template with those fields as required, and if so how do I do this? 

3. Add custom fields for that data? 

 

And if anyone has a great JIRA book, I would love to get a suggestion.
