Hello everyone!
I've got a problem with collecting weekly reports in an impact statement format from my teams. In order to do that, I would like to create a form that asks guided questions to hit every point of the weekly report (Accomplishments, Issues, Awareness).
In a perfect world, the form/question flow would work like this (Accomplishment used for example):
- Accomplishment 1:
- What is the accomplishment? -> What does the accomplishment impact? -> Who was involved with this accomplishment? -> On form submission, answers are concatenated together to form a rough impact statement with all topics covered.
The first question would be on a "slide" and would not progress until answered. When answered, the question would progress to the next "slide", so on and so forth.
What I've done so far:
- Rev 1: Create a site for team leads to edit and drop in the requested info. Inputs are reviewed and edited for clarity and content.
- Rev 2: Created a form for them to fill out and the form is read by a form report. Inputs are reviewed and edited for clarity and content.
The problem with this is that since there are no leading questions, inputs are open to interpretation. We've tried training, one on ones, and self edits, but we'd like this to be more automated.
We end up going through these guided questions and have gotten much better results when asking these leading questions.
So my ask is this, are there any resources or addons that allow for "slide progression" or a Google Forms Quiz style setup/addon I could use to ask these guiding questions, one at a time?
Thank you for any and all help!
