User Access Request Form

Question

I have a use case where I need a form filled out to request access to a application. When the form is filled out it opens a support ticket in JIRA with the form data. The users that will be filling out the form will NOT be existing customers in JIRA and I don't want to have them sign up. Essentially when they submit the form, it creates their customer account in JIRA.

Does anyone know how this could be accomplished

Thank you for the response, but my goal was to have the form available without having to log in. What I would like is the public facing form that a user fills out, once they submit it, it creates the user account (similar to what happens when someone emails a ticket in that does not have an account).

If you are using Jira service desk as you have this request tagged then you can go to Project settings > Customer permissions and set it up so any customer can submit requests.  Then you can create the form in the portal and they will create their account when they login or if you are submitting the form through email just make sure it comes from their account and it will create the account or you can use the API to create the issue and create the account.
