When I log in - my account shows an old Jira someone connected me to. How do I disconnect it?

Welcome to the Atlassian community.
You have to contact the administrator of that Jira instance to ask them to remove your access.
It is not possible for a user to "leave" or revoke their access to a Jira environment they do not administer.
