removing auto inserted fields in a Help desk form

I would like to remove these field shown in the image attachments but cannot find anything online about it, thoughts?

Did you get a solution to this. How do we hide the "Contact us about" and "What can we help you with?" fields

Welcome to the community.  You shouldn't delete those fields.  The layout are the key elements the of the Portal UI.  You can however remove the grouping of your request forms, thus your users will just see a listing of all the request type forms in the portal UI.

The Contact us about section displays all the request types groups defined in your JSM project.

The What can we help you with section displays the all the custom request type forms associated within a group as defined in your JSM project.

Thank you for the reply, however in this case we want nothing BUT the one form, no changes no swapping to other pages. The image is the top part of the page ABOVE the form we have created using the forms function.
