The Forms in Confluence no longer show a respondent's name after they have submitted a response. For example, I created a form with voting options for them. I see the vote, but not who voted. I did not select for anonymous responses. Previously, names were shown.

I may be wrong, but based on your description, I suspect you're using a Marketplace form app inside Confluence rather than a native Confluence feature. Atlassian still documents surveys and forms in Confluence as functionality provided through Marketplace apps rather than something built directly into Confluence itself.
That is important because whether respondent names are recorded, displayed, or anonymized is controlled by the form app providing the form, rather than by Confluence's own permissions.
Could you check the form in edit mode and let us know the name of the macro or app that appears when you select it? A screenshot of the form while it's being edited would also be enough.
Once we know what's actually providing the form, we can tell whether the missing names are expected behavior, a setting that changed, or possibly a regression.
