Is there a way to use Automation rules to change page status when the page has been reviewed, and the reviewer has added a signature with the Meaning / Role of "Approval"?

If you are still looking for a solution, eSign Document Management & Training supports electronic signatures with meanings for regulatory compliant environments (e.g. FDA Part 11). After signing it can trigger Automation webhooks to update Confluence page status.
Might be worth a try.
