Depending on the case, it could be related to an issue in the instance or some configuration not being applied. So, to get a background of that, do you record when you started to face this issue? For example, after having the add-on upgraded.
Also, can you check if the atlassian-confluence.log <Confluence Home/logs directory> display anything related to the questions symptoms?
