How do I add a comment to the Issue while logging work in Tempo Timesheets?

How do I add another field to this window? (This is NOT the log work window)

Marie,
you are not able to enter a comment on the worklog dialog screen that will be saved to the JIRA issue (if I understand you correctly).
You might use other add-ons (script runner) in order to achieve what you are looking for. This is out of our scope.
