Forms for Jira and Confluence

Hi everyone,

I’m looking for a solution (or an app) that allows working with forms seamlessly across both Jira and Confluence. Specifically, I’d like to:

  1. Create forms in one platform (Jira or Confluence) and use them in the other.
  2. Share, manage, and analyze form submissions easily.
  3. Possibly have a way to automate workflows or task creation based on submitted forms.

Does anyone know of an app or workaround that supports these features? Any recommendations would be greatly appreciated!
