We're using Jira Work Management (Cloud). I'd like to be able use Forms to create Epic-level (hierarchy level 1) Issues. However, when I click to create forms in the project, it only allows for the selection of Story-level (hierarchy level 0) Issues.
Is there a way to setup forms to create level 1 Issues?

According to the documentation JWM Forms can be used to create only standard level (hierarchy level 0) issues.

There is a change request about supporting Epics for Forms to which you can add your vote:

Since you have a Premium subscription, then you should be able to create an Advanced Form (this is the same type of forms used in Jira Service Management). Go to Project Settings and see if you see Advanced Forms on the left side menu.
If not, open a support ticket with Atlassian and ask them to add that for your instance.

Thank you, John! Advanced Forms does not appear on the left side menu of the Project Settings. I will also try opening a support ticket as you suggested.
