Yes, it is possible to add custom fields to forms in JWM provided that these fields are previously created.

Yes, Jira has always had a "custom field" function - you can add as many fields of all sorts of types (select, user, date, text, etc) to any project.

You will need to talk to the administrators of the project if it is team-managed, or your Jira administrators if your project is company-managed.
