Hi,
I have been asked to add a select list custom field and require a customer description if the default list option is not used.
How would I make it so if the customer doesn't select the default option, it will open a box for them to give the reason.

I believe you will need some application for such thing. I'm aware of two, which could help you with this, but probably there are more of them:
