How do I give each service request separate fields?

I am trying to create service requests, all of which require separate fields, but when I make changes to the screen it affects all service requests. The service requests have different fields selected but this makes no difference.

Any help would be greatly appreciated.

Hi Dale:

In this case, you will need to create different Request Types, so you can customize each request type's form independently via the EDIT FIELDS option.  It seems that your request types' forms are all using the same form.
