Credentials don't persist while configuring Crucible with JIRA

I have configured my crucible server with the JIRA instance and it seems all right initially; I can see the fisheye and crucible details in the Source and Review Tab of a JIRA issue. But after sometime the link gets broken. When I go the application link and see Outbound Authentication; it shows un-configured. It simply asks for UserName and Password which I had entered previously and saw getting persisted. The JIRA version is JIRA (v5.1.5#784-sha1:6c72993)

I will attach a word document that I prepared from your help to guide the IT team and will really appreciate if you could go through that and point me out any mistake
