Looking for a solution to provide a weighted scoring system

I am trying to create a Confluence page that collects a multiple choice answer to a set of questions (this is not a survey, just needs to be filled out by any user). The data collection and scores should be persistent.

Each question uses radio buttons (or similar) to select a single answer only to each question.

Each answer has a different weighted value associated with it.

Then the score at the bottom (or top) of page is simply a weighted total of all the answers.

i.e.

Q. What is your age?

  1. <20 (weighted value 1)
  2. 20 – 40 (weighted value 3)
  3. >40 (weighted value 5)

Q. What is your highest education level?

  1. High School (2)
  2. Bachelor (6)
  3. Post Grad (7)
  4. Masters (10)

Then if the user selects answers 2 and 4 respectively, then the report would show weighted score of 13.

Does anyone know if there are plugins that can do this or a combination of macros to achieve this?

Has anyone tried to do something similar and can share their code?

No solution here, I have reverted to using MS Excel....
