Create Confluence Intake Form and Convert to Jira Ticket & Email

Is there a way to create an input form/page in Confluence to capture information (intake form) and then convert that information into a Jira issue/ticket in a project.  Can we also simultaneously email the information to someone(s)?

This is not possible with standard Confluence, but it is certainly possible (and a lot more) with the app we develop and support - ConfiForms

We have Enterprise Confluence not Standard Confluence.   Is it possible with Enterprise Confluence.

As far as I know - No, not possible with any Confluence edition

You can create a form in Jira using Smart Forms for Jira and embed it on a Confluence page.

The app allows you to:

  1. Design Forms: Create dynamic, customizable forms.
  2. Embed in Confluence: Add the form to Confluence (Guide).
  3. Automate Jira Issues: Configure automatic Jira issue creation upon form submission (Guide).
