Collecting user feedback and filing to under the right page

Hello, I know my options are limited as the Server and plugins are no longer for sales.

I want to collect patient's health issues by filling a form in Confluence. There will be checkboxes (Task List available under Insert more content). This is the only thing available. It seems that I can't have a free form textarea (as in HTML), no radio buttons, no other user input options.

Also, if available, I want to place an empty text (as in HTML) for patients to fill in their name. Once patient submit this form, it will be filed as a new page under this patient's name. If patient's name is not already available, create a new page and place this form under this page.

I can write some code (may be I can learn how to write Confluence macro). 

Is this possible at all? How much of this can be done?

Hard to say since not sure which version of Confluence do you have and what plugins are available, but looking only OOTB you might check and create a Page Templates..

Just not sure if your patient would have ability to create the page and how you would like to handle visibility so that one patient would not see other pages..
