unable to upvote few "Questions" in the installed version.

Currently we are evaluating Confluence Questions and have the version : 1.0.239

I can upvote few Questions and some fail, and if clicked multiple times, the negative voting shows up that goes down upto -2.

As per the logs, this is what I can see..

Sorry, received the email update on your comments today - Sep 15, 2014. Maybe one more bug with notification scheme.

Anyway we ended up dropping CQ due to various bugs at the time of our evaluation.
