hi, @Wim Abts You create the Smart Jira Forms with some field logic and automatically add it to every issue of some request type. So, it might have a question if customer is happy with resolution. If he clicks NO, the Form will expand with additional fields. Once submitted, you will be able to see the result in the issue.
