See https://confluence.atlassian.com/jirakb/migrating-from-jira-server-to-jira-cloud-779160826.html

The basic tasks are

Upgrade the server to the latest server version (7.13 at the time of writing)
Export to a backup
Import the backup on Cloud (which is not 7.13, it's Cloud "latest")
