A user is trying to login and receives the error: Oops, you've made a malformed request.

I have a user who has been logged out and now, upon trying to log back in receives the error: 

Oops, you've made a malformed request.

I have disabled 2FA, uninstalled Chrome, re-installed and I get the same message. The user is able to log in through Safari, but that is unsuitable for his work environment/flow.
