In the workflow, when the issue is transitioned to "done" status, we want a resolution screen to display. How do we do this?
@Rita Allen, welcome to the Community. You will want to edit the workflow associated with the issuetype. Edit the transition to Done and add a screen to the transition. In that screen you will want to offer the Resolution field for the user to select the appropriate reason.
