I would like to have the date on which the hours are logged added to the e-mail notification.

To the following information:

Worklog entry: [no of hours]

Change by: [name]

Remaining Estimate: [hours, minutes]

Time spent: [all time spent on an issue]

I would like to have added the follwing:

Work date: [date on which the hours are actually spent]

Is this possible and how should I go about this?

There were some extra characters added to the hyperlink i posted..

Try copying https://confluence.atlassian.com/adminjiraserver071/customizing-email-content-802592932.html and https://developer.atlassian.com/server/jira/platform/adding-custom-fields-to-email/ to notepad and remove the extra characters after .html and email/
