Is it possible to create a custom form with three columns on the customer portal?
I'd like there to be three columns for the following checkbox, Name & description.
Has anyone had any luck with this or could recommend away to go about this?

It is not possible with out of the box Jira Service Desk. You could see if this add-on might help with what you are looking for though.
