Duplicate user appear in UpRaise company directory

As the title mention, there is a number of users (30%+) in the UpRaise directory are duplicated. And I have no idea what causes the bug.

I have tried to manually inactive the duplicate account in the database, but after some time, the system automatic active back the duplicate account.

This bug is quite annoying and confuses the user. Hope that there is someone who can help with it.

Below is the sample user list in the system.

UpRaise Version: 5.1.1

Upraise Access: JIRA group-based access

 

Please let me know if there is additional information needed to diagnose the bug.
