Issue collector form misbehaving after Submit clicked

Question

I'm trying to use an issue collector. The basics are working - the user provides the feedback, clicks on Submit and a Jira issue gets created. However, the form just sort of "fades out" and it isn't at all clear that the submission has succeeded. As a user, you are left wondering what to do other than just click on Close. Looking at the network traffic, it looks like a POST operation takes place, sending the data from the feedback form, and the reply looks like this: but nothing is getting displayed on the form.

I don't remember seeing that. To be honest, I've been having so much trouble with trying to get the issue collector to work that it has become an on-again, off-again type of project, so I cannot clearly remember where I've got to with it now.

I'll resurrect it and see what it does now.

You don't get a success message in green notifying you that it will close itself in 5 seconds?

I've been encountering the same issues lately. Is there anything to do with recent JIRA updates? Or Javascripts? Would like to know how to solve this as well.
