Carry over story reporting

I would like to create one dash board item to show how many story is getting carry over from one sprint to next sprint. So that we can do a RCA and improve the process based on the root cause.

In the VisualScript for Jira marketplace application, we have a built-in velocity report.  It includes story points committed, delivered, added, dropped and rolled.  It shows these metrics over a period of time, and broken down by sprint.

Hi, 

Is there a way to add  to this report (or dose anoth exist) that shows a brakedown of what type of 'stories' have been added to a sprint eg story,  bug or spike. 

Also can you display the results and show it across prior completed sprints?
