Checklist Automation Via Rules Is Not Working When A New Card is Added To the Board

Hi!

Every month, I need to complete some account recs and I'm trying to transition my work to Trello.  I have a rule set up as follows:

Here's my template card with the checklist:

Yet, the account rec checklist that is on my board is blank.

Any help would be greatly appreciated!
