Hi, I am facing the same issue. I am OnDemand with 10 users license however when I try to do a upgrade on user tier, I receive a message that says that it will be automatically upgrade to match but I can't create anymore due to the button being disabled.
You haven't told us what user authtication system you're using (OnDemand's user management, your own Crowd, interal Jira, external LDAP etc), and you haven't told us what you've tried and what doesn't work.
If you can't answer those directly, then the simple question to get started is "What do you get when you try Admin -> User management type options"?
