Task list due date in Confluence is not working

Hi,

I am running Confluence 5.7. When I try to create a task on a page I see a few issues:

• The tasks is prefixed with a bullet point and not a check box.
• When I try adding a due date using "//" the calendar is not invoked. 
• In addition, I cannot use the keyboard short cut "[]" to create tasks as it does not function.

Out of curiosity I ran a test in another Confluence instance that we have (but with version 5.5.6), and above functionality fully works.

I'm with the same problem.

It happened once and stops after a while (we restarted the serve ate some point between)

And now it happended again.
