How to create a JIRA issue from a form in a Confluence Page?

At my organization, we are currently using Confluence and looking into JIRA. I would like to know if a JIRA issue can be created with a custom form embedded in Confluence.

For example, in Purchasing Department's Confluence space they have a page called 'Submit a Travel Request'. Once the user is on that page they can fill to form that will collect custom information such as name, destination, start date, end date, hotel costs, etc...

Once they submit the form it should create a new JIRA issue in Purchasing Department JIRA project.

Not sure if this is answered somewhere already, but I've been working with the issue of how to create Jira tickets from a Confluence form, and here are some key notes + possible solutions or workarounds:

(Applies for Jira Cloud)

Notes:

Ticket creation forms are available natively only with the Business Project and Service Manangement project types.
The forms work slightly differently on both, but to summarize:
- Business Project forms are simpler and look nicer, but have very little customization possibilities. Works well for very simple ticket creationg (little to no custom fields)
- Service Management forms are the opposite: Slightly less easy and nice looking, but allow more customization.
Software Project type does not currently support built-in forms,
Using the Issue Handler is not possible either, because currently Confluence Cloud version does not support html code input.

Possible solution:

Use either Business Project or Service Management project type when creating project, then use the built-in "Forms" tab to create a form, then paste the link to that form to a Confluence page. (I can add more step-by-step instructions if needed, but the "forms" is pretty easy to find and pretty intuitive to create in my opinion.

If you must use the Software Project type anyway, then here is one workaround:

Create a Business Project to work as a "Forward HUB" project
Create a form using that project and add it to a Confluence page.
Create automation to the "Forward HUB" project, to create a duplicate ticket to another project of your choosing (the intended project), then delete the one created by the forms.

This creates couple seconds of extra delay as the main downside, but in theory this same "Forward HUB" can be used to "forward" to multiple different projects, as long as you have a custom label, issue type, etc. to distuinguish the identificaton for the automation.

(Again, if you want more step-by-step instructions, just ask, but this is what I've found out so far to be the available options with Atlassian Cloud, without using 3rd party apps or plugins.
