How to Create Custom Field of Text or Attachement - or Checklist

Question

Is it possible to create a custom field that only displays text or one that has an attchement?

What I am trying to accomplish is, for a particlar project, have a field that will display the same text - or contain the same document - for every issue that is created.

The document is simply a checklist, so if there is a way to create checklist field, that would work too.
