How do I find Jira Form 31 and Form 25

From 22 November 2024 Requests must be submitted by using the below “Jira Portal” links

Hello @CARTER Mike 

Welcome to the Atlassian community.

Those URLs look like links for completing Forms that will generate issues in Jira Business/Work Management projects. However in my system the ?from at the end of the URL is not present.

You should be able to submit your issues using those URLs after removing the ?from at the end of the URL.

If that doesn't address your question, please provide more details.
