How do I add a field to my screen if it is not available on the default screen or custom fields.

It sounds like you'll need to create it. Why do you think it exists currently if it isn't in the custom field list?

I suspect this is related to " https://answers.atlassian.com/questions/15292430 " and it's starting to make me think the question needs to be clarified - what do you think is missing here?  What field are you looking for?  

If you go to edit a screen (as an admin), you are given a list of all fields in the system, so if you think something is missing from there, then it's because you have not added it to the custom field list...
