Getting an error as, "Issue can't be created due to the error/s [ Phase Created is required. ]. What is the expected value for "Phase Created"

The issue has been resolved.
Phase Created is part of "Document Update Request" (Issue Type) - screenshot attached for reference.
While uploading issues from csv a default value was auto-assigned on Map Values-->Target values in JIRA field as "13084" and this resulted in creating "Document Update Request".
Deleting the default value and proceeding with issue upload was successful.

It looks like the Phase Created field is required to create a new issue. The field is probably a pick list (dropdown) and the value in the CSV is not one of the values in the list of the dropdown. Therefore, the value cannot be put in the field, and therefore, the issue cannot be created.

You need to look up that custom field and see what the valid values are for it.
If you click the Create button and put in that project, then just click the drop down for that field.
