Custom required field by label / checkbox field

Question

Problem: while making deployment developers forget to make changes in the database.

Solution: If developers add special label ("db-deploy") or check item in the checkbox ("Requires db deploy"), developer have to fill in custom text field ("Steps for db migration").

Please advice me how to do it.

Condition on transition -> Post function to inspect value and pop up screen on value found to capture inputs.
