How to create a custom Issue field with a selection that opens a hidden form?

I'm trying to create a custom issue field with the options yes or no. If no is selected nothing changes.

If yes is selected I want to display 5 relevant questions. The questions will be the following field types. 1,  check box 2, text box 3, user selector 4, text box and 5, number text box. 

What would be the best way to accomplish this? I've tried using a cascading select field but it's limited to selection and doesn't offer the text box options. I also searched online for similar issues but haven't had any luck.

In order to achieve this you will need an app.

It could be Scriptrunner with Behaviours,ProForma or Dynamic Forms.
