I want to create a field in by JSM form by the name of Contribution Upload .
Now this field should contain a textline in this manner :

where 'Please follow this link to upload your contribution(s)' would be line to url text, clicking on it will lead to a further URL.
However, this field should not be editable at user's end.
Can someone suggest how can I achieve this ?
If you are prepared to enable HTML in the field descriptions one can use the description text to contain an anchor tag.

Use Forms(Proforma) function in Service Management to build your forms.

You can use info panels to add information for users.
