How can I connect answers from a survey displayed on a website to JIRA without using Google Forms?

My team is launching an internal-only website where people in our company can make requests. We'd like to automate the process by having the survey answers generate a JIRA ticket. Our company cannot use any Google products. Does anyone know a survey service or plugin that can do this?

You have several options here:

  • Use the JIRA Issue Collectors - on which best suit what you need, a survey form or some form for bug reporting without actually logging into JIRA! You can embed this to any website easily with just few simple steps (https://confluence.atlassian.com/jira/using-the-issue-collector-288657654.html). 
  • Create your own script and fronting application and call the JIRA REST API behind the scenes.
