Show text output result based on options selected in multiple dropdowns

Hi,

I would like to do the following in Confluence Server

Dropdown 1 

Dropdown 2 

Result 1 (appropriate result appears based on options selected in dropdown 1 & 2)

 

Dropdown 3

Dropdown 4

Result 2 (appropriate result appears based on options selected in dropdown 3 & 4)

Final Result 3 (based on result 1 & 2)

This was created in excel sheet and results were calculated using excel formulas. I was unable to do this in confluence and could not find any marketplace add-ons that could do this.

Anyone knows how can I do the same or any add-on that can achieve this?

There are several "forms for confluence" add-ons in the marketplace that are worth  a look.

Thanks for the response. I am trying "forms for confluence" by Adaptavist. How do I calculate and display the final result as in the above example based on the first 2 dropdowns.

Dropdown 1: Yes (Value:5) or No (Value:0)

Dropdown 2: Yes (Value:5) or No (Value:0)

Dropdown 3: High (Value:10) or Medium (Value:5) or Low (Value:0)

Result : I would add up the values and display the a text message based on the total value of dropdown 1, dropdown 2 and dropdown 3

Is this possible in "Forms for Confluence"?
